    Pana-Pacific is pleased to support a $200 rebate offer on the purchase of a complete RoadLog Base or Plus electronic logging device (ELD). The promotion is only valid on purchases made between January 8, 2019 through June 30, 2019 through
    Pana-Pacific. Below defines what a complete RoadLog Base or Plus system purchase would entail:

    *Please note that a driver key is also required for each individual driver that will operate an ELD-equipped vehicle for the system to function properly. While supplies last, Pana-Pacific is offering these free of charge upon request. To request the FREE driver key(s) please contact our customer service group at telematics@panapacific.com

    Instructions for Submitting a Rebate Request:

    The following items must be submitted to eva.lopez@panapacific.com in order to receive reimbursement:

    1. A Completed W9 Form (FOR U.S. TAX IDs) or a W8 Form (FOR CANADIAN TAX IDs): If you do not already have a copy that you can provide, please download the blank form below, complete it and include it with your rebate request. We are required by law to have this information on file if we distribute funds in the amount of $600 or more to any individual or entity in a calendar year. (A downloadable copy of a blank W9 can be found at https://www.irs.gov/pub/irs-pdf/fw9.pdf) For Canadian customers without a US tax ID, you will need to provide or complete a W-8 form. (A downloadable copy of a blank W8 can be found at https://www.irs.gov/pub/irs-pdf/fw8ben.pdf)
    2. Proof of Purchase: A dealer invoice or transaction receipt is sufficient.
    3. Payable Information: Provide the complete business name that the reimbursement check should be made out to.
    4. Mailing Address: Provide the address to which the payment should be sent.

     

    Please allow 6 to 8 weeks from submission for your payment to be sent by mail.
